Planning for your departure from Drake is an important part of your treatment. In fact, you'll start talking about discharge planning on the very day you arrive. With a good understanding of your recovery goals, we can help you work toward them from the start.

Your length of stay

No matter what you've been told, it's hard to know how long your stay will last. Your length of stay is based on several factors, many of which we can't control. Your case manager is advocating for you and will work to get you all the benefits to which you're entitled.

Preparing to leave

Just as every patient's care is different, every discharge situation is different as well. Based on your medical needs and personal situation, your Drake Center team will work with you to determine your best transition.

We will also help you identify and navigate the barriers to your transition goals. This means your discharge plan may include community re-entry sessions to address real-life issues like mobility techniques; time in one of our transitional living suites to help you practice your skills in a home-like environment; or a home safety visit to evaluate your home's accessibility.
